ASQ Information Summary sheets are now available in an alternate format. These alternate sheets use a scoring chart design that does not incorporate shading to differentiate results.

Programs who have purchased ASQ-3 in English may complete this form for free access to the alternate files. Files for ASQ-3 in Spanish and ASQ:SE-2 in English and Spanish will be available later this fall.
